Travel & Required Forms

The forms found here are HCC required student club forms for activities. All off-campus events will require some or all of these forms to be completed. Day events will not require the "Overnight" forms, but will require the Emergency Contact Form and the Waiver of Liability Form. In addition, all students upon engaging with the Pi Theta Chapter in any way, will complete the Picture and Information Release Form.

Student Overnight Guidelines
All students must complete this form for overnight travel.

All students must complete this form for all activities.
Waiver of Liability Form
All students must complete this form for all activities.
Picture & Information Release
All students will be required to sign this form, or accept that it is their responsibility to remove themselves from group opportunity photo shots.
